Note: The form does not apply to all 50 states. Select your request type.
Enter your first name, last name, and email address (we recommend using a temporary email address).
Enter your request details.
Click the “I’m not a robot” checkbox.
Click the “Submit” button.
5. Click the email confirmation link
USA TODAY will send you an email confirmation link to verify your identity.
Go to your email inbox and find the email from USA Today.
Click the “Confirm Email” button.
