How to Remove Yourself from Equivax via Opt-Out Prescreen

Complete the Opt-Out / Opt-In request form:

1. Go to their online form athttps://www.optoutprescreen.com/selection.

2. Scroll to the bottom, select “Permanent Opt-Out by Mail”, and click “Continue”.

3. They will ask for your name and address. Your social security number and date of birth are not required, and we recommend not giving it out.

4. Fill out the security letters and click “Confirm”.

5. Print out and mail the “Permanent Opt-Out Election form” to:
