Opt out of SalutaryData in two steps. 

1. Go to the SalutaryData website and click “Do Not Sell or Share My Personal Information” in the footer 

Go to http://salutarydata.com/ and scroll to the bottom of the page. 

Click “Do Not Sell or Share My Personal Information” in the footer.

2. Fill out the form

You will be redirected to a page with an online form. Fill it out.

Select your request.

Enter your first name, last name, work email, personal email, current employer name, employer street address, city, state, and zip code. 

Enter your reason for request (for example, “privacy”).

Click the “Submit” button.

You can also make your SalutaryData opt-out request by emailing privacy@salutarydata.com or calling 1-866-451-4868 (toll-free).

Alternatively, write to Privacy Officer, Salutary Data, 29 Locust Ave., Lexington, MA 02421. 

If you submit your request by email or snail mail, provide your name, address, phone number, and any other identifiers you think will help SalutaryData verify your identity.
