Scroll to the ‘Opt-Out’ section.
Click the hyperlinked “Opt Out Form.”
3. Click “Please submit your opt-out request here instead”
You will be redirected to a page with an online form.
Fill it out if you’re a resident of the United States but not a resident of California, Colorado, Connecticut, Utah, or Virginia.
California, Colorado, Connecticut, Utah, and Virginia residents should click “Please submit your opt-out request here instead.”
4. Fill out the form
You will be redirected to a page with an online form. Fill it out.
Enter your first name, last name, street address, city, state, and zip.
Enter your SSN.
Enter your date of birth.
The other fields are optional.
Click the “I’m not a robot” button.
Click the “Submit” button.
Your DecisionLinks opt-out request was submitted and should be processed within 72 hours.
