3. Click “Civis Data Subject Request Form”
Scroll to ‘How to Exercise Your Privacy Rights.’
Click the hyperlinked “Civis Data Subject Request Form.”
4. Fill out the form
A new tab with an online form will open up. Fill the form out.
Enter your email.
Select the state you reside in.
Click the “Next” button.
On the next page, select who you’re making the request for.
Click the “Next” button.
On the next page, enter your first name, last name, street address, and city.
Select your state.
Enter your zip code, email address, and phone number.
Click the “Next” button.
Select your request type. You can select multiple options.
Click the “Next” button.
Click the attestation checkbox.
Click the “Submit” button.
Your CivisAnalytics opt-out request was submitted.
