2. Fill out the form
You will be redirected to an online form. Fill it out.
Start by selecting the request you’re making (“Remove my information from the database,” “Tell me what lists my name is on,” or both). The “Remove my information from the database” option is already pre-selected, but you can select both options here.
Enter your first name, last name, birth year, address, city, state abbreviation, zip code, and email address.
You can also enter your phone number, but this field is optional.
Click the checkbox “Yes, I am human – not a spambot.”
Click the “Submit” button.
3. Confirm your request
CompleteMedicalLists will ask you to confirm that you want your record deleted if CompleteMedicalLists finds it in their database.
Type in your name to confirm that you want your record deleted.
Click the “Yes, I am human – not a spambot” checkbox.
Click the “Submit” button.
Your CompleteMedicalLists opt-out request was sent.
CompleteMedicalLists will search its database for your information and, if found, delete it within 30 days of receiving this form.
