1. Go to the Deloitte website and click “Do Not Sell or Share My Personal Information” in the footer
Go to http://deloitte.com/ and scroll to the bottom of the page.
Click “Do Not Sell or Share My Personal Information” in the footer.
2. Click the hyperlinked “Personal Information Request Form”
Under ‘Covered Activities (excluding cookie-based Targeted Advertising),’ click the hyperlinked “Personal Information Request Form.”
3. Click “Request”
A new tab will open with information on how to submit a personal information request.
Scroll down and click the “Request” button at the bottom of the page.
4. Click “Myself”
Deloitte will now ask you if you are submitting a request for yourself or on behalf of someone else.
Select the relevant answer.
5. Fill out the form
You will now see a form you need to fill out. Fill out all the mandatory fields.
Enter your first name, last name, email address, street address, city, and state/province.
Select your country and enter your postal code.
Select your relationship with Deloitte.
Select your request. You can select multiple requests.
Provide any additional notes you think might help with your request (you can leave this blank).
Click the checkbox beside ‘I confirm that I am submitting this personal information request for myself.’
Click the checkbox beside ‘I have read and accept the Privacy Notice and Terms of Use.’
Enter the CAPTCHA text.
Click the “Submit” button.
Your Deloitte opt-out request was submitted.
You will receive a request number – save it for your records.
Deloitte should get back to you within 15 – 45 calendar days.
