Go to https://www.driveniq.com/ and scroll to the bottom of the page.

2. Fill out the form
You will be redirected to a page with an online form on the left. Fill it out.
Enter your first name, last name, and email.
Click the checkbox to confirm that the name and email address you entered in the form are yours.
Click the checkbox to opt-out from further opt-ins for 12 months.
Click the “I’m not a robot” checkbox.
Click the “Submit” button.
Your DrivenIQ opt-out request was submitted.
