1. Go to the Healthcare website and click “CCPA Personal Information Request” in the footer 

Go to http://healthcare.com/ and scroll to the bottom of the page.

2. Fill out the form

You will be redirected to an online form. Fill it out.

Select your inquiry.

Enter your first name, last name, email, zip code, and phone number.

Click the “I’m not a robot” checkbox.

Click the “Submit” button.

Your Healthcare opt-out request was submitted. Within the next 48 hours, Healthcare will send you an email with a link you need to click to verify your request.

3. Click the email confirmation link

Go to your email inbox and find the email from Healthcare.

Your Healthcare opt-out request was confirmed. 

You will see your personal information request case ID. Healthcare will also email you this ID. Save it in case you need it in the future. 

Your request should be processed within 45 days.
