Opt out of Indivizio in two steps. 

1. Go to the Indivizio website and click “Do Not Sell My Personal Information” in the footer 

Go to https://www.indivizio.com/ and scroll down to the bottom of the page. 

Click “Do Not Sell My Personal Information” in the footer.

2. Fill out the form

You will see a page with an online form. Fill it out. 

Select whether you’re a customer or authorized agent. 

Enter your first name, last name, email address, zip code, street address, city, and state.

Select your privacy request. You can select multiple options. 

Click the “Next” button.

Review the information you entered.

Click the checkbox to confirm the information is correct.

Click the “Submit” button.

Your Indivizio opt-out request was submitted. Indivizio should respond to you within 45 days.
