3. Fill out the form

A new browser tab will open up with an online form. Fill it out.

Enter your first name, last name, email, and ZIP code. 

Leave the optional fields empty. 

Click “Yes” under “Under penalty of perjury, I declare that the above information is true, correct and that I am the person or an authorized agent of the person, whose name appears above.”

Click “Both” under “What type of request would you like to make?” This will allow you to opt out of the sale or sharing of your personal data and limit the use of your sensitive personal information.

Type in the CAPTCHA code. 

Click the “Submit” button.

You will now need to confirm your identity via email.

Go to your email inbox and find the email from J. D. Power.

Note: If you don’t see the email from J. D. Power, check your spam folder. 

Click the “Confirm Email” button to verify your identity.

J. D. Power will respond by email within a few days. The email will include a link to a privacy portal where you can view updates on your opt-out request.
