Leave the box under ‘I would like to opt-out of my data being shared with third parties’ checked. 

Leave the box under ‘I would like to request access to my personal information’ checked (if you want to see the information Media Source Solutions has on you).

Enter the CAPTCHA.

Click the “Send” button.
