Go to https://optimalfusion.com/ and scroll to the bottom of the page. 

Click “Do Not Sell or Share My Personal Information” in the footer.

2. Choose your request and fill out the form

Scroll down a little until you see an online form. 

Select your privacy request (Request to know, Delete my information, or Do not sell or share my information). 

Fill out the form. 

Enter your first name, last name, postal code, and email address.

Check the box beside “I hereby swear, declare, and affirm under penalty of perjury under the laws of the State of California that the information I supplied is true and correct, that I am either (i) the consumer identified in this request, or (ii) authorized to make this request on behalf of the consumer, and the consumer identified in this request is a resident of the State of California.”

Click the “Submit” button.
