2. Fill out the form

You will be redirected to a page with an online form. Fill it out.

Select your request type. You can select multiple options.

Enter your first name, last name, mailing street, mailing city, mailing state, mailing postal code, email, and phone.

Click the “I’m not a robot” checkbox.

Click the “Send” button.
