You will be redirected to an online form. Fill it out.
Enter your first name, last name, address, and city, and select your state.
Then, enter your zip code, email address, email address again (to confirm it), and phone number.
Type in “YES” to certify you’re the individual to whom the above information belongs to, acknowledge that the information above is correct, and confirm you want your personal information deleted and not sold.
Your opt-out request was submitted.
